Advanced Planning Documents

 We often take a great deal of time when it comes to planning meaningful events in our lives like graduations, weddings, birthdays, or retirement.  However, talking about death is never a pleasant subject, yet it is a fact that all of us must face one day. Advanced planning gives you a unique opportunity to make informed choices, using logic and reasoning, rather than be influenced by emotion.  These arrangements are just as important as your other advanced directives such as a Will, Living Will, or Durable Power of Attorney for Health Care.

Once you have decided to use cremation, the next step is to make certain that your family is aware of your wishes.  We have developed a method to do just that and it only takes a few minutes.  For your convenience we have provided a Vital Statistics Form that may be downloaded and printed, or you may click on the form and complete it prior to printing.  This form is the first step in pre-planning your cremation without having to pre-pay.  This form allows you to record your desire for cremation and provides vital information necessary for cremation authorization, as well as the Death Certificate.  The second form allows you to list your Personal Preferences and Requests.  This allows you to give detailed instructions concerning a memorial service, final placement of the cremated remains, scattering or burial instructions, urn selection etc.  Once we have received your documents, we will confirm that your arrangements are on file by sending you a Cremation Pre-Arrangement Certificate and Emergency Identification Card.  There is no cost to set up this file and you are not obligated to use our services should you change your mind.  The cremation services may be paid at the time of death with a valid insurance policy, cash, or credit card.  The price would be at the prevailing rate at that time. However, if you decide to pre-pay, the price is frozen at today’s costs.  We offer payment plans to meet the needs of every family.

We feel this better prepares a family rather than a notation in a will.  A will is sometimes not read or accessible until after a funeral, thus overlooking any specific wishes one may have.  Most importantly, your family will not be faced with having to make countless emotional and financial decisions at a time when logical choices are seldom considered.

After you have reviewed and completed the Vital Statistics Form and Personal Preferences and Requests page, please return the forms so we may create your file and issue the Cremation Pre-Arrangement Certificate and Emergency Identification Card.  Should you be an Honorably Discharged Veteran, you may want to provide a copy of your discharge papers (DD-214).  This would allow us to file for VA Benefits, obtain your Veteran’s Flag, or to arrange for an Honor Guard should you choose.  At your request, we will be happy to provide copies of your arrangements and contact information to the person who will be handling your affairs.  Please mail your completed forms to:
